CROWNS
$1000 Vouchers are given to tribal members for 1 crown every 12 months. If you have insurance then the Voucher can cover the remaining amount after insurance has paid their portion.
REMOVEABLE PARTIALS
$900 Vouchers are given to tribal members for a removeable partial denture every 3 years. If you have insurance then the Voucher can cover the remaining amount after insurance has paid their portion.
INVISALIGN / ORTHODONTICS
$800 Vouchers are given to tribal members for orthodontic dental work.
NIGHTLASE Snoring Treatment/ TMJ Laser Therapy
$800 Voucher can be used for this treatment
MYOFUNCTIONAL THERAPY
$800 Voucher can be used for this treatment
FAQs
$800 Vouchers are given every 12 months to be applied towards any dental treatment. Fillings, INVISALIGN, TMJ treatment, etc. You do not get one for each type of dental work. The voucher can be spread out and be used towards several different types of treatments. The voucher will not exceed $800 in a 12 month period.
There are vouchers specifically for Crowns and Removeable Partial Dentures that do not conflict with the $800 miscellaneous dental voucher.
You can call Choctaw Patient Relations to request your Voucher and they will fax it to the office.
